RE interleaving of custom highlights with native highlights: per resolution to #4595, we're not going to allow it for now but will reassess if we find use cases in the future. The rest of this issue seems to be about exposing native highlights via the OM and changing how they stack relative to one another. That should probably be solved by the css pseudo spec, rather than the highlight API spec, so changing labels. (Feel free to change back if someone feels otherwise.)
