Re: [csswg-drafts] [CSS2] require all headings to have hardcoded IDs (#4867)

FWIW: this adds IDs for the top-level headings of most pages of the form `sec-[filename]`. This is perhaps not ideal. 

Note that the following filenames already exist as IDs: cascade, changes, colors. grammar, sample.

The only three top-level headings which already have IDs are: box-model (for box.html), the-page for (page.html), generated-text (for generate.html)

Happy to change the rest of the IDs provided we can come up with better options.

An initial suggestion might be:

 * about.html: about
 * intro.html: intro
 * conform.html: conform
 * syndata.html: syndata
 * selector.html: selector
 * cascade.html: ??? assigning?
 * media.html: media
 * box.html: box-model (unchanged)
 * page.html: the-page (unchanged)
 * visuren.html: visuren
 * visudet.html: visudet
 * visufx.html: visufx
 * generate.html: generated-text (unchanged)
 * colors.html: sec-color?
 * fonts.html: fonts
 * text.html: text
 * tables.html: tables
 * ui.html: ui
 * aural.html: aural
 * refs.html: refs? biblio? or follow Bikeshed and references?
 * changes.html: sec-changes?
 * sample.html: html-stylesheet?
 * zindex.html: zindex? stacking-contexts?
 * propidx.html: propidx? or follow Bikeshed and property-index?
 * grammar.html: sec-grammar?
 * indexlist.html: indexlist? or follow Bikeshed and index?
