Is the difference between underline and strikethrough that an underline is drawn behind the text and a strikethrough in front, i.e. a drawing order difference? Then the only time when strikethrough and underline are indistinguishable is when they are the same colour. My guess is that is probably the majority of the cases in actual use so worth avoiding.
