+1 to excluding descendants. 

He's an (unfortunately complicated) example. On Shopify.com we use child elements to expand the hitbox so it's forgiving when hovering horizontally on menus. Since the outline includes these elements, when tabbing through the menu, you end up with odd pyramid-like outlines.
