Actually it's necessary to use counter-set to make my last example work, which I realised on the way home last night - I was hoping to edit it before anyone noticed, but too late. Now edited with comment. However it won't help. counter-set has the same effect on scoping as counter-increment but was added to the spec in 2011, and I believe the text in css-page is largely unchanged from the original in about 2009.
