This issue was closed, no change, by the CSSWG in Seattle. 
`::first-letter` rules determine the extent of `::first-letter`, this 
is clearly specified; furthermore `text-combine-upright` forbids 
combination across element boundaries, so the 13 cannot combine. If 
the author wants this effect, s/he needs to wrap the desired 
characters in markup and apply `text-combine-upright: all`.
