Improving the Design Guide contents

I'd like to discuss my thoughts on the Design Guide in this week's TF 
meeting and also think about what might be done in the Content Usable 
release timescale.

As noted in our planning page I've been looking at ways to further 
improve the Design Guide content for Designers and developers. I've also 
kept in mind our plans for making the guide available as both a note and 
web version.

In GitHub Issue 154 I've noted the kinds of changes I'd like to see. 
These range for simple editorial to adding new sections for the web version.

Concentrating on the Pattern "Clearly Identify Controls and Their Use" 
as an example, I have provided:

* comments in a Google document copy of the current text
