Usage of capital letters - upper case

Please forgive me for raising an issue that I am not sure we have really discussed, but has come to my attention as I am working on MOOCs and has an impact on readability as well as accessibility.

The use of capital letters as part of the templates used in a MOOC framework where longer titles, sections and URLs appear in upper case and cannot be changed by learners to a lower case alternative. 

This issue appears to be against UAAG 2.0 1.4.6   but only under a AAA level https://www.w3.org/TR/UAAG20-Reference/  despite being a known issue for most readers, especially for those with reading difficulties and visual impairments. 

I have found the draft WCAG 2.0 guidelines had this under 3.3 (extended) https://www.w3.org/TR/2003/WD-WCAG20-20030624/   but I cannot find the statement in the latest versions?   

All I could find was under Readable 3.1. https://www.w3.org/WAI/WCAG20/quickref/#qr-content-structure-separation-understanding   1.4.8 and advisory techniques 3.1. and that was very vague!

"Using upper and lower case according to the spelling conventions of the text language (future link)"

Does anyone know if there are any AA level guidelines dealing with the subject of capitals or use of upper case?    (I cannot believe it is not in there somewhere and I am sure I am missing something very obvious)
