Review PRs on Processing Model by 10 Dec 2015

Hi Group,

We've agreed to place a priority on reviewing Paul Adenot's pull requests
relating to the AudioContext processing model that underpins AudioWorker
and much else in the spec.

Please attach comments directly to the PRs or to lines in the commits.

The deadline for feedback is EOD Thursday 10 December 2015.
