This issue has been previously reported as:

My understanding is that this issue makes it impossible to reliably string together a succession of audio sources without glitching. Which in turn seems like a showstopper issue for games with seamless background audio environments.

I feel that this is a consequence of the following issue with sub-sample accurate scheduling that I have just now logged:
