[web-audio-api] Use of param in example 4.5.4 (#259)

The following issue was raised by the W3C TAG as part of their [review of the Web Audio API](https://github.com/w3ctag/spec-reviews/blob/master/2013/07/WebAudio.md)

Where does `param` come from in example 4.5.4? It's not marked constructable as per 4.5 and there doesn't appear to be a `create()` method for it documented anywhere. It's a ghost ;-)
