Decision on CfC: Rename Personalization TF as WAI Adapt

Colleagues:

This call for consensus received overwhelming support, and no direct
opposition. There was only the question of whether this name would be
more restrictive to the Task Force's work in the future, and the
proponderance of descussion suggests it would not.

Consequently, this CfC is agreed to as a consensus decision of APA. The
