Call for review: Update rules to ACT Rules Format 1.1

Hey folks,
This pull request adds a version number to all rules, moves the assumption
and accessibility support version to the background, and updates the
"outcome" definition. Once this change is merged all rules should be
updated to version 1.1 of the ACT Rules Format.
