I'd suggest moving the "recommended nonce" paragraphs to an appendix, 
and maybe having two sections to the appendix: one for where the server 
doesn't care about replay, and one where it does, with a suggested 
nonce structure for each and procedure to use with that nonce.  They 
really aren't part of the "specification", which what the section where 
they currenly are is titled.
