XSV up-to-date, major bug fix, upgrader

The online version of XSV [1] is now up-to-date with the revised PR
version of XML Schema [2].  A serious bug affecting schema documents
without DOCTYPEs has been fixed.

An online XSLT-based tool [3] which upgrades XML Schema documents from 
the 2000/10/XMLSchema to the 2001/XMLSchema namespace, implementing
all the changes from the one to the other, is also available.
