Tweaks to XENC Requirements in Preparation for Publication as NOTE

I believe the final resting home for Requirements now is a Note. So I've 
done a few tweaks to the req document to get it ready:

- Updated some of the references.
- Tweaked the box which said we want feedback on attribute encryption and 
say it was discussed but there was no push back during last call.
- Removed the other discussion boxes.
- Removed the instance validity text about detached encryption and CDATA 
sections and added the option of application metadata (headers) that can be 
used to indicate to the app how to restore the document.
- In the requirement not to invent a transform processing model, I added a 
link to recent discussion (with the compression as the example).
