Can errors in the overal SOAP message be faulted as if they were errors
in the SOAP Body?  Suppose I find an unqualified attribute name in the
Envelope?
The discussion of detail in section 4.4 says MUST NOT talk about
headers, but MUST talk about Body, and is silent about Envelope.

Perhaps this should be added to the open issue?
