Most of the draft presumes that simple links are now always inline and
extended links are always out of line. This I hold to be a Good Thing.
The text of the issue "Inline attribute" says so.
However, the last paragraph of clause 5.1 continues to talk about
simple out-of-line links, and should be removed.
