R120 URI-references, input for editors

Here's the updated specification for inclusion in Part 1, non-mormative
appendix. (See attached file: URI-References.html)

It includes the following changes:
- Added note on non-backward compatibility with WSDL 1.1
- Adopted use of namespace URI as basis for URI-reference
- Added support for extension elements
