[CSS21] Issue 198 and box tree

Great (although the specific issue is additionally that 9.5.2 refers to 
the element tree when instead it needs to refer to the box tree).

Please note that the box tree does need explicitly formalizing, since in 
addition to being needed at that part of 9.5.2 and again in Appendix 
E,[2] it's also likely needed at another point in 9.5.2 as described in 
[3].  No doubt there are other places too.
