The CSS WG decided as follows on David Baron's comment[1] about what 
happens to margin/border/padding/text-decoration where an element is 
broken:

  - Add to the end of 13.3.1:
    "When a page break splits a box, the box's margins, borders, and
    padding have no visual effect where the split occurs."
