[CSS21] out-of-date references

Many references in CSS 2.1 Last Call Working Draft are out of date.
I trust editors can figure out correct references by themselves, so
I only briefly describe which entry should be updated.

B.1 Normative references

  link broken
  new edition available
  new revision available
  RFC 1700 is replaced by an on-line database (see RFC 3232)
  link broken
  10646-1:1993 is way out of date, 10646:2003 is the latest version
  new edition available
  obsoleted by RFC 2616
  new minor revision available

B.2 Informative references

  second edition available
  it's a REC now
  newer draft (PR) available
