QAH issues for f2f

Here is a start and enumerating the QAH issues and filling in some proposed 
resolutions:

One bit that hasn't been handled in any detail yet derives from the Action 
Item that Mark executed, to replace the use of fuzzy terms like "QA 
deliverables" with more specific references.  This generated some email 
comments, and appears to rub up against some more-than-editorial issues 
about scope of our work, etc.  There's a placeholder and reference in [1] 
... have a look and think about it some before the F2F.

I may get a little more done before F2F, e.g., on the last section.
