It might be worth for you to check out the efforts done by the HR-XML organization, their DTD for exchanging international address and name information. It might gives you some ideas to create a good DB schema. You'll find it at http://www.hr-xml.org/schemas/doc/cpo/index.htm (free registration will be required to access the documents).
