Sect. 8.3.4:
  inside <message>, add an '"/' between "stop" and ">".

Chapter 11 (thoughout): invalid MIME:
  missing double quotes around parameter values:
    11.4: mulitpart/related example: first line
      - type=application/xml    +type="application/xml"
      s/start"=/start="/
  missing empty lines between part headers and part bodies:
    11.4: multipart/related example (4x)
    11.5: multipart/form-data example (3x)

Appendix G.3:
  on <s:survey>, the "http://www.example.com/survey" namespace is defaulted,
but
children
  of <s:survey>, as well as <s:survey> itself, continue to use the s:
prefix.
