Here's another go:
- CR55: DONE
- CR58: was DONE yesterday (but not listed in my email)
- CR59: DONE
- CR60: was DONE yesterday (but not listed in my email)
- CR61: DONE
- CR63: DONE
- CR66: DONE
- CR68: DONE (fixed 2.2.1 to 2.3.8, i.e. both fault propagation rules 
and meps)

This fixes all outstanding Part 1 & 2 issues albeit the following three:
- CR26: Isn't the existing text clear enough?
- CR41: Isn't this covered already by the last paragraph of 6.5.2?
- CR57: It looks to me like an xs:string already. (It's "type" which is 
an xs:QName.)
