Thank you very much for this cogent advise [1]. The document is well
researched, written, and much appreciated. My hope is that it will be
a guide for the HTML 5 Working Group.

I have started a "headers" issue page on the group's wiki [2] per the
editor's request [3] and incorporated many of your points as well as
linked to your document.
