I do believe browser extensions are the tool. However, thank you for the
bookmarklets.

Alastair's basic model is correct. We don't want authors to write their own
alternative layouts. It is lots of work and often doesn't do the job.

We need to promote this from a WebAIM point of view -- Design with
accessibility in mind. Given that there are problems.

3rd party inclusions can be hard to reach from a browser extension.

I am also concerned with runtime positioning, especially from 3rd party
inclusions.

If you look at the WCAG 2.1 doc. That blue banner on the left is almost big
enough to be problematic, and I can't make it go away. What did W3C do?
