Additional XML Signature Second Edition and explanation draft updates

I have completed some additional editorial updates to XML Signature,  
Second Edition available in the Editors Draft redline [1] and noted  
in the explanation document [2] which I also updated.

(1) XML Signature, Second Edition updates:

+ Updated contact information for Joseph Reagle.
+ Corrected two incorrect uses of xml-example style in section 7.3
+ Fixed link to C14N reference in section 6.5.
+ Fixed text for updated XML URL and added trailing slashes, a  
correction to previous XML reference update.
+ Updated MIT link to csail instead of lcs, and updated SAX URL to  
avoid redirect.
+ Fixed validation errors.

(2) Explanation updates:
+ Added note regarding change to Joseph Reagle contact information
+ Added notes about MIT link update and SAX link updates.
+ Fixed all section items to have uniform titles
+ Fixed validation error.
+ Adjusted links to Overview.html instead of review.html
+ Fixed broken reference link for C14N11
