Document Updated

I've made minor adjustments to the document[1]:

* Added some padding/etc. to make requirements/use-cases easier to read.
* Changed the document dates to the publish date 04/04/2006
* Added the note about the requirements-to-uses-cases table being
   incomplete.

Still to be completed are the ordering changes.
