Can I just agree with you instead?  :)

A couple of comments:

* By having an element, authors can decorate the elements with
   other annotations or allow documentation for them.

* I'd rather name 'p:here' 'p:document'

* Since p:internal and p:external are small and compact
   elements, I don't see much expansion in the expression
   of the input/output.

* Automatic generation of pipelines just got bit easier
   because the use of the internal/external/here source
   was separated from the pointer.  ...but only a tiny
   bit.
