4414 finished

I've checked the primer into cvs with all the changes.  I did a few more
editorial changes as well, such as adding an ignorable + optional
section.  The  table is in there too, and very illuminating.  Note, I
don't know how to do table headings yet, so there will probably be one
more small edit done to make it look a bit better.
