Updated WSDL 1.1 Element Identifiers

I've done many of the updates requested including:
- uncommented the canonical form section
- All but one of PaulC's comments
- Umit's comment about example.

I'm having some problems getting the linking done properly, particularly
that specref doesn't seem to work to table/captions, and I'm talking
with PLH and others about it.
