Missing xml namespace prefix declaration (WSDL, editorial)

I don't think I need to raise an explicit issue about this since it's
primarily eidtorial and the change needs to occur in the WSDL document. 

WSA 1.0 - WSDL Binding Editor's Copy 2005/03/22

Example 2-1 does not declare the XML namespace prefix "wsaw" used in
line 8 of the example.

I suggest adding the missing declaration:
xmlns:wsaw="http://www.w3.org/@@@@/@@/addressing/wsdl"
