Re: [webcomponents] Custom elements disconnected from a document should not be upgraded (#419)

I spent a decent amount of time thinking about this, and there really does seem to be no alternative, apart from retaining a strong reference to all undefined potentially-custom elements forever, which seems like an obvious no-go.
