Re: [streams] Is Node's "unshift" necessary? (#3)

Turns out @acolwell needs this for the MSE spec, see step 9.3 of https://w3c.github.io/media-source/#sourcebuffer-stream-append-loop

Also I think with the `ExclusiveReader` API some of the optimization concerns above (that I see upon re-reading this thread) are probably addressed.

I'll try to work on this soon, probably week of 2015-02-02 since TC39 is next week.
