[ET TF] Form to help generate database entry/e-mail

Dear ET TF,

Please find an initial form to assist you communicate with tool vendors and generate database entries for tools.

When you enter information about a tool in this form, you have two options:
1. Generate an e-mail that you then need to send to the respective tool vendor or developer
2. Generate an entry for the tool in the database (in RDF/XML format)
