updated: "Process 101" final requirements and rough concept draft

In addition to the edits made in response to Judy's comments, I also changed:

* change 1>2 in "Size: 2 printed pages" - with the navigation printing and the diagram, no way to keep to 1 page
* added: "Note: Be careful to keep short & simple, not try to address all related issues."
* moved items from [open} list down to [done]
* added references to minutes and such at :

including previous draft at:
	http://www.w3.org/WAI/EO/Drafts/w3c-process-old1

I'm done for now. Open for discussion:
* How is the overall approach?
* Is it about the right length?
* Does it cover the right topics?
* Does it meet the requirements?
* Ideas for next revision?
Note: it's only a rough concept draft, the writing is not at all refined yet

If you have comments soon, I'll try to incorporate them before sending to EOWG.
