To complete ACTION-211 [1] I have applied the resolutions for ISSUE-62 [2]
and ISSUE-63 [3].

Note that the application of the change for both issues is slightly
different to the proposals because :

1. The word 'Definition' appears at the beginning of the bullets instead of
at the beginning of the appended text.  This is necessary in order to
include the fault definitions into the existing assertions without term
spec. generation failures (caused by duplicating the termdef in the
assertion table).  The alternative would be to document the fault
definitions in new protocol assertions.
2. The new faults have also been added to the XML schema in Appendix B.
