v4 of test cases spreadsheet

Attached is the latest test cases spreadsheet (v4). I made the following changes based on 11/20 and 12/11 decisions:

- removed noSchemaNV from feature sheet and feature-test-map sheet
- removed baseUriCheck from feature sheet and feature-test-map sheet
- removed baseUriReq from feature sheet and feature-test-map sheet
- cleared the contents of rows 31, 62, 66, 67, 83, 90. I did not delete the rows since this would make future references to our tests in the context of Cosmos' responses more difficult. So for now, the rows are empty but not deleted.

I think the cell formulas on the feature-test-map are incorrect. Could someone please check that? The formula references cells on the 'test' sheet. This cell reference is incremented by one in each row on the feature-test-map - this would happen if you did a "Fill Down". However I believe that the formulas in all column D and column E cells in 'feature-test-map' should be referencing all the rows in the 'test' sheet.
