[XSLT 2.0] media types and fragment ids and the document() function

I find the text in section 16.1 concerning the processing of fragment
ids somewhat perplexing, especially the text for error XT1160.

Since the commonest media types text/xml and application/xml do not
define any rules for interpreting fragment-ids, then in most cases the
processor must either report an error or recover by ignoring the
fragment-id.

This, I think, will be contrary to user expectations - the common
expectation is that the fragment-id should be interpreted as an ID
value.

I think that perhaps there should be special mention of these media
types, and specify that the rules of the XPointer framework should be
considered as applicable to these media types
