placeholder rank in this is based on observed browser behaviour and the
concept derived from the ARIA accessible name calculation algorithm that

is always the accessible name source of last resort

Note that the use of placeholder for accessible name calculation is a
repair mechanism, it's used when there is no better source for accessible
name.
