Review of EL++ section (ACTION-118)

This discharges my action, 118.

The changes seem fine. The global restriction is no harder to read  
than the "nonstructural" restrictions in the syntax doc. We'll  
probably want to make them a bit more readable to more people, but  
again, whether in the primer or here is open, IMHO.
