BP edits

Hi all,

I've made some edit modification to the BP document.
- Remove one of the TODO as we discussed emails.
- Remove the Revision Log (since i was in the file I could do that AI of yours Felix)
- Fixed some formatting in examples.

All back in CVS.
Editors: make sure to get the latest dita\EX-dita-its-relating-1.xml file along with the BP XML.
