This is a very good question and highlights an omission in the current
version of the EXI specification. Section 8.5.3.1.3.2 ComplexType Grammars
[1] should include some additional text to add productions of the form:
 
    Content[i,0] : 
        CH Content[i,0] 
 
when the {content type} of a type definition is mixed.
 
Thank you for pointing this out! This omission will be corrected in the next
version of the specification.
