Please lose the DOCTYPE -- header should be:

<?xml version="1.0" encoding="UTF-8"?>
<html xmlns="http://www.w3.org/1999/xhtml" xmlns:epub="
http://www.idpf.org/2007/ops">

Perhaps it be good to compare to a know good sample -- you should check out:

https://github.com/IDPF/epub3-samples/releases/download/20170606/moby-dick.epub

After download just change ".epub" to ".zip" and unzip... then will will
have a known-good example to follow.
