tabindex, axis and headers/id Bindings

The following example was adapted from Techniques for WCAG 2.0.

Note 1: This is a dummy form. There is no explicit submission mechanism, nor is there an actual form to be processed. It is intended to demonstrate navigational and contextualizeing mark-up.

Note 2: the "title" information provided in the original example for each input element has been removed, so as to test support for the headers/id binding mechanism as defined by HTML 4.01 by assistive technologies, in particular screen readers and self-voicing applications. The headers/id binding has been supplemented by the use of the axis attribute, as suggested by the HTML 4.01 Technical Recommendation's section on categorizing cells.
