The following example was adapted from Techniques for WCAG 2.0.

Note 1: This is a dummy form. There is no explicit submission mechanism, nor is there an actual form to be processed. It is intended to demonstrate navigational and contextualizeing mark-up.

Note 2: the "title" information provided for each input element in the original example has been preserved, leaving it the sole identifier of each input field's purpose. The expected result is that the contents of the title attribute should be manifested visually, by a ToolTip onFocus, and as well as expressed multi-modally by assistive technologies, in particular screen readers, refreshable braille displays, and self-voicing applications.

Note 3: This is, by far, the most labor-intensive means of providing an explicit binding, as each title value has to be uniquely defined so as to uniquely identify each form control. It is also the most reliable means of multi-modally defining contextualizing text for a form control, as most assisstive technologies and user agents support the exposition of the title attribute defined for an interactive portion of a document instance's user interface.
