This is entirely managed by the formatter and out of the hands of the 
stylesheet writer.

This sounds like a problem with the formatter you are using.

There is no such formatting object and the extent is dynamically calculated.

If there is no room on the page for the footnote reference area to grow, 
the formatter is allowed by 6.10.3 to use a following page.
