Oh, you're right, ":" is disallowed in a QName.  So the prefix would
have to be something like "code." instead:

        < . . . xml:id="code.12345" . . . >
        < . . . xml:id="code.foo" . . . >

Admittedly this is not *quite* as pretty as "code:", but I think it's
still pretty clear to a human reader.  Might this be good enough?
